Billing Information

When you send a specimen to Reliance Diagnostics for laboratory analysis, you should also send a completed Test Requisition Form, which is included with each specimen collection kit we provide. On the Test Requisition Form, indicate whether the patient, you the client, Medicare, Medicaid or an insurance plan should be billed for the analysis of this specimen. If the form is not completed correctly, we may not be able to process your order.

In many cases, payment can be made after the requested laboratory test has been performed. We will contact you if pre-payment is required.

As a courtesy to you, Reliance Diagnostics can also file any reimbursement claims for our services to your insurer, Medicare, and Medicaid. If you the client would prefer to be billed for services you have ordered, we can send your office a monthly invoice.

Of course, coverage can vary based on the patient’s specific health plan benefits. Patients are responsible for payments, co-payments, co-insurances, and deductibles if applicable.
